OSHA is fining Alcoa’s Davenport Works as the result of an employee death. Forty-five-year-old Robert Leech, of Wheatland, Iowa, was the father of five. He died last July 16th while doing maintenance on some machinery that, according to a federal report, was not properly secured. The report says the machinery was not pinned in place after being positioned.

The report says Alcoa did not specifically address procedures for controlling movement of the machinery due to gravity. OSHA has cited the plant for six serious safety violations and a proposed fine of $37,800. An Alcoa spokesman says the company is reviewing the report.
